Lifeline Disclosures
About Lifeline
Lifeline is a government assistance program supported by the federal Universal Service Fund. Lifeline service is non-transferable, only eligible consumers may enroll in the program, and the program is limited to one discount per household. Consumers who willfully make false statements in order to obtain the benefit can be punished by fine or imprisonment or can be barred from the program.
Proof of eligibility
You must show proof of eligibility through a qualifying program or household income. Phone Government and the Lifeline National Verifier may request documents.
One per household
Only one Lifeline benefit is allowed per household, whether for wireless, home phone or broadband service.
Usage requirement
If your free Lifeline service is not used for 30 consecutive days, we'll notify you and you will have 15 days to use the service to avoid de-enrollment. Usage includes making a call, sending a text, using data, or responding to our notice.
Annual recertification
You must recertify your eligibility every year. Failure to recertify will result in de-enrollment. See recertification.
Changes in eligibility
You must notify Phone Government within 30 days if you no longer qualify, if another household member receives Lifeline, or if you move. Call 1-800-633-7797.
